  • - The Centers for Disease Control and Prevention (CDC) teamed up with the National Association of City and County Health Officials (NACCHO) to develop the new Million Â̾ÞÈËÊÓƵs® in Municipalities Toolkit, a tool to help health departments and municipal organizations implement Million Â̾ÞÈËÊÓƵs® at the local level. It was designed to engage city, county, and regional partners to adapt the Million Â̾ÞÈËÊÓƵs® structure and framework to the needs and assets of their community. It includes 5 modules that can be used, individually or as a package, to tackle capacity needs related to goal-setting, partnerships, communications, and evaluation and monitoring. Each module includes a list of online resources that may be used to inform the design of a municipal’s evidence-based approach to prevent heart attacks, strokes, and other cardiovascular events. We encourage you to explore how this tool may be used to inform your cardiovascular disease prevention efforts.
  • For a model local health department and community toolkit for improving hypertension outcomes, especially in smaller and more rural communities, review the  from Million Â̾ÞÈËÊÓƵs WI and the Association of State and Territorial Health Officials (ASTHO) Million Â̾ÞÈËÊÓƵs Learning Collaborative.
